The undersigned do hereby authorize First Baptist Church of Alhambra, its Pastors, or their Representatives as agents for the undersigned to consent to transport above mentioned minor to church activities
There may be pictures taken at church activities. Photography or videos may be used to highlight events on social media, printed materials, website, and other FBC publications. The undersigned gives authorize FIRST BAPTIST CHURCH OF ALHAMBRA, ITS PASTORS OR THEIR REPRESENTATIVES, permission to photography minor and uses the image or video for above stated reason. If you wish to not allow minors image taken please notify the staff.
Church activities include, but are not limited to hiking, swimming, and/or high-risk activities. There are risk of physical injury or harm from participating in high adventure activities. I, or my minor, voluntarily elect to participate in the activities and assume the risk of
injury or harm that could result from participation. On my own behalf and that of my personal representatives and heirs. I hereby release FBC its officers, employees, and agents from all liability for any injury or harm to me (or my minor) from participating is said activities, whether the injury of harm is caused by negligence of FBC or otherwise. I have read and understand this liability of release.
The undersigned do hereby authorize FIRST BAPTIST CHURCH OF ALHAMBRA, ITS PASTORS OR THEIR
REPRESENTATIVES as agents for the undersigned to consent to x-ray examination, anesthetic, medical, dental, or assigned diagnosis or treatment and hospital care for the above minor which is deemed advisable by and rendered under the general or special
supervision of any physician and surgeon, licensed under the provision of the MEDICINE PRACTICE ACT of of any dentist under the DENTAL PRACTICE ACT, at a hospital or elsewhere.
In the absence of parent or guardian, the above-mentioned agent is authorized to make decisions concerning the health and welfare of this minor.
First aid and non-prescription medication will be administrated at the adult leader’s discretion with the following exceptions.
